If your patient is inactive or needs support or guidance with increasing their level of activity, then get them on the Green Prescription programme
Practice Nurses or General Practitioners can submit a referral using Medtech:
- From the "Documents" box on MedTech enter "GRx" to bring up the pre-populated GRx referral.
- Complete any additional details
- Click "send to" at the bottom of your screen, choose "Email Recipient" and email direct to grxsupport@sportcanterbury.org.nz or print off and fax to Sport Canterbury (03 387 0284).
Other Health Professionals need to complete the Health Professionals Referral Form, and fax to Sport Canterbury (03 387 0284).
